For prosthetics, you must be cleared by your primary/referring physician before we start any prosthetic treatment plan. Physician documents and a prescription are required to initiate the process. This prescription/referral is also required by all insurance companies.
For orthotics, you must have an active prescription and current physician documents from a primary or referring physician to allow us to better assist you. The information will help formulate our treatment plan and ensure that we provide the necessary orthotic device. This prescription/referral is also required by all insurance companies.
If you need a NEW prosthetic or orthotic device, you may need to schedule an appointment at our facility and with your primary doctor. This will ensure that we establish proper documentation and obtain an active script to provide to the insurance company.
Please bring your ID, insurance card(s), and the prescription/referral with you to your scheduled appointment. This will allow a staff member to scan these into our system and save them for future use.
If you are a new patient or your information has changed please print our Patient Information Sheet to fill out and bring with you to your appointment. This will make the check-in process quicker and easier.
If you need to cancel or reschedule an appointment, please give us a call as soon as you know you will not be able to make your appointment time. One of our staff members will be happy to assist you.
We accept all major credit cards as well as cash or check. For current patients needing to make a payment, we do accept credit card transactions over the phone. Please contact one of our offices and they will be happy to help you
